PCN Head of Operations Becky

I am the Head of Operations for CCH PCN, I have 35 years NHS Primary care experience and am enjoying working with the 4 different general practices in Central Chelmsford that make up our PCN.

I joined the team at the beginning of April 2024 to manage staff and all the business requirements needed as part of this enhanced service. This involves making sure that we have the right staff in the right places spread across the 4 sites to ensure all patients get an equitable service from any of the 4 practices.

These extra staff are Paramedics, Musculoskeletal specialists, Pharmacists, Pharmacy technicians, Care co-ordinators, GP assistants, Social prescribing link workers, a Health and well being coach and Mental health practitioners. It is a goal of the NHS to make sure you see the right person for your medical need at the time and these staff have become invaluable in their help and assistance to our 4 General practices. We also run the extended access clinics which are held every evening up to 8pm and on a Saturday 9am to 5pm.

  • Folloing patients through their cancer journey - from referral to post treatment support
  • Build care plans for care home residents
  • Complete lifestyle referrals, e.g Weight management and smoking cessation
  • Signpost carers to support
  • Help Mental Health patients access support

Enhanced Access Service Anna - Operations Manager

Our Enhanced Access Service offers out of hours appointments for routine care needs as listed below. These are provided between 6:30pm and 8pm on weekdays and 9am-5pm on Saturdays at Baddow Village Surgery CM2 7EZ

General Practitioner (GP) Appointments:

  • Routine consultations
  • Follow-up appointments
  • Chronic disease management

Nurse Appointments:

  • Wound care and dressing changes
  • Vaccinations and immunizations
  • Cervical screening (Pap smears)
  • Chronic disease management (e.g., asthma, COPD)

Preventive Health Services:

  • Health check-ups
  • Screening for conditions such as hypertension, and high cholesterol
  • Advice and Referral for Lifestyle support (e.g., smoking cessation, weight management)

Pharmacist Services:

  • Repeat prescription services
  • Medication reviews and adjustments
  • Chronic disease management (e.g., diabetes, asthma, COPD)

 

Family Planning and Sexual Health Services:

  • Contraception advice and services
  • Coil (IUD) insertion and replacement
  • Contraceptive implants insertion and replacement

Joint Injections Clinics:

  • MSK steroid injections for joint pain relief

  • Home Visits
  • Care Home Ward Rounds
  • Care planning for Care Home residents
  • On call support

We can assess and treat any number of health conditions including those relating to:

  • Breathing Difficulties
  • Stomach Problems
  • Skin breakdown
  • Head injuries
  • Short- or long-term medical conditions
  • Safeguarding and welfare Checks

We can also provide more general support and will always liaise with the on-call doctor to ensure the correct treatment path for you

  • Medication Queries
  • Drug Formulary Changes
  • Medication Updates
  • Medicines Optimisation – where we find the most beneficial and cost-effective treatments to manage your conditions

Like Clinical Pharmacists, we also update your medication from Discharges and Clinic Letters.

Social Prescribers are a group of professionals looking to address people’s social and clinical needs.

We provide telephone and face-to-face appointments to discuss a range of needs. These can include:

  • Autism and Other Neurodivergences
  • Dementia Care
  • Carers Support
  • Food Bank
  • Bereavement
  • Loneliness

For both professionals and members of the public, it is worth getting in touch with your local social prescriber, accessed through the GP or GP reception team, to see if they can support you, a family member, or your patients (professionals).
